Requirements for Miscellaneous Services

a) New Passport


  1. Duly filled Application Form No.1. 
  2. Two recent photographs (size 35mm X 45mm) with plain white background and the dress in preferably in colours offering contrast with background for clarity.
  3. Original passport of the applicant.
  4. Photocopy of the Civil ID of the applicant



c) Police Clearance Certificate
  1. Duly filled application form.
  2. Original Passport is required.
  3. Passport copies of the applicant.
  4. One recent passport size Photograph (35mmX45mm)
  5. If applicant is on a commercial visit visa, provide an offer/contract letter from the Employer duly attested from Kuwait Chamber of Commerce and Ministry of Foreign Affairs.
  6. Company’s registration Licence copy(Etamatokiya)
  7. If the applicant wants to go to any other country, the applicant’s stay in Kuwait should be minimum 3 years.
b) Change of Address
  1. A copy of any of the current utility bill like water, electricity, telephone, House Tax and nationalized bank account passbook copy.
d) Any other service (Specify)
